Consent for the Processing of Personal Data

In accordance with the Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data (hereinafter "Regulation") and in accordance with the Data Protection Act No. 101/2000 Coll. (hereinafter "Data Protection Act"), as amended and effective from May 25, 2018, I hereby give consent to BetterSide.ai s.r.o., with its registered office at třída 1. máje 3414/11A, 690 02 Břeclav, ID: 21287325, (hereinafter "Company") as the data controller

to process

my personal data to the extent of name, surname, phone number, email (hereinafter "Personal Data")

for the purpose of processing inquiries and orders received through the Company's website;

for a period of 10 years.

The Personal Data is provided to the Company voluntarily.

The Personal Data I provided is accurate and true.

Personal Data will be processed both automatically and by Company employees.

The Company has informed me that, under the conditions set forth in the Data Protection Act, I have the following rights against the Company:

a) the right to information about the processing of Personal Data;

b) the right to rectify or complete inaccurate Personal Data without undue delay;

c) if I discover or suspect that the Company is processing Personal Data in violation of the Data Protection Act, I have the right to, under the conditions provided by law:

          I. request an explanation;

         II. request the Company to promptly rectify the situation, particularly by blocking, correcting, supplementing, or destroying Personal Data, if it does not conflict with other legal regulations;

        III. seek reasonable satisfaction in accordance with applicable legal regulations if my personal rights have been affected; and

        IV. claim compensation for damage from the Company if the damage was caused to me by the Company;

The Company further informed me that, under the conditions set forth in the Regulation, I have the following rights against the Company:

a) the right to erasure of my Personal Data in cases provided for in the Regulation;

b) the right to access my Personal Data;

c) the right for the Company to restrict the processing of my Personal Data if:

          I. I contest the accuracy of the Personal Data, for a period allowing the Company to verify the accuracy of the Personal Data;

         II. the processing is unlawful and I oppose the erasure of the Personal Data and request the restriction of their use instead;

        III. the Company no longer needs the Personal Data for the purposes of the processing, but I require them for the establishment, exercise, or defense of legal claims;

       IV. I have objected to the processing pursuant to Article 21(1) of the Regulation, pending the verification whether the legitimate grounds of the Company override my legitimate grounds;

d) if a likely case of a personal data breach would result in a high risk to the rights and freedoms of natural persons, the Company will notify me of the breach without undue delay.

The Company has informed me that in order to exercise my rights, I can contact the Company in writing at its registered office address or at the email address info@betterside.ai.

I also have the right to contact the Office for Personal Data Protection or file a complaint with the Office for Personal Data Protection.

I hereby confirm that I have been informed of my rights and their protection under Sections 11, 12, and 21 of the Data Protection Act, as amended, and under Articles 13 - 22 of the Regulation.
